In addition to this, the owner/IT chief of Sinclair is scamming software programmers. He has a job ad posted on indeed.com for a senior software developer. Upon applying for the job, he emailed asking for a number. I gave him my number and he called and did the "screening". He then said that I'd have to undergo a "Test" to get the job. I got suspicious right-away but wanted to be sure and asked him to send me the test. Instead of a normal software interview quiz/test, this guy sent me the following, which is an entire program that runs on the web. He figured he'd scam me into writing out the entire program which would take about 8-10 hours and then run. Here is the scam test he sent:

********************************************************************************************************************************

Introduction
Thanks for coming in and participating in this sample code project. First off, this isn’t a test. The only goal of this project is to give a real life sample of your code, so we can get a feel for how you write and organize your code as well as your programmatic approach to the objectives. There should be nothing tricky about this project. If anything doesn’t appear completely straight forward, don’t hesitate to ask. If you think of questions after you begin, don’t hesitate to ask. Finally, aesthetics are NOT important, so feel free to just concentrate on the code.

Setup
Imagine a company is running a project to collect data based on the 3 simple questions.
• Was the store clean?
o Yes
o No
• How friendly was the salesperson ?
o Very Friendly
o Friendly
o Unfriendly
o Very Unfriendly
• Did you receive a receipt?
o Yes
o No

These questions and the corresponding responses are held in two normalized tables:
• [tblQuestion]

• [tblQuestionResponse]

When the questions are answered, the data should be stored in 2 other normalized tables:
• [tblDataHeader]. This is the master data table that holds the survey id, the respondents I.P. address, and a date time stamp.

• [tblDataRecords]. This holds a record id, and 3 foreign keys (idSurvey, idQuestion, idResponse)

Objectives

Populate Data
Your first objective is to build a form that dynamically displays these questions, allows the respondent to answer the questions, and stores the results in the respective data tables.
Report data
Next, build a small report (table) that displays the results. You have two options here:

1. First (easier) just query the data, and display the results in a flat table that looks like this:

2. Alternately (and a little more difficult), query the data, and display results in a table that reflects (by question) how often the respondent chose a “top box” response. Top box responses are identified by the [isTopBox] flag in [tblQuestionResponse].

Should you attempt this option, your final report should consist of a table that looks like this:
